Subject outline

In this subject students will explore a range of chronic health conditions as a major contributor to morbidity and mortality in Australia and the multidimensional approach required in managing such conditions. Students will: (i) examine models of collaborative care in the management of chronic illness and disability, and (ii) reflect on the role of nurses in partnership with individuals, their family and carers in the planning and delivery of care that can assist to empower an individual to manage their own health.
