Subject outline
Chronic diseases are becoming more prevalent in aging societies. In this subject, students will integrate core strands of theory knowledge and practice with contemporary health needs of individuals, communities and populations. Students will develop knowledge and skills in the treatment and self-management of chronic diseases. Students will critique of models of health, theories of health behaviour and their application to specific case scenarios, while considering a range of presenting conditions. An interprofessional, person-centred and evidence-based approach will be adopted.

01. Critically evaluate partnerships of care, considering the patient and the role of health care professionals in chronic disease management
- Activities:
- Workshops with reviews of clinical cases and discussion of therapeutic partnerships between health care professionals to achieve patient-centred care in chronic disease management. Management plan.
02. Research, critically analyse and apply theories to develop chronic disease management plans in response to authentic case studies.
- Activities:
- Workshops with reflection of the physical, emotional, cultural and psychosocial impact of chronic conditions and critical review of models of care to encourage behavioural and lifestyle changes and maximise participation of individuals with chronic diseases. Management plan
03. Analyse the barriers and facilitators of effective chronic disease management to synthesise strategies to maximise patient outcomes.
- Activities:
- Within workshops, in teams, identify and lead a class discussion on contextual factors influencing chronic disease management. Management plan.
04. Apply existing theories of human behaviour, lifestyle management and communication to generate enhanced interaction with individuals with chronic diseases in the context of clinical scenarios.

- Simulated patients / actual patient interviews, peer review/feedback. Video and reflective diary.
