Subject outline
Chronic diseases such as diabetes, asthma, arthritis, osteoporosis, cancer, and chronic cardiovascular conditions are becoming more prevalent in 'aging' societies. In this subject students will develop knowledge and skills in the prevention, treatment and self-management of chronic diseases that reflects the National Chronic Disease Strategy. An interprofessional, person-centred and evidence-based approach will be adopted.

03. Critically analyse and reflect on partnerships of care
- Activities:

04. Research and critically analyse evidence and theories to develop chronic disease management plans in response to authentic case scenarios
- Activities:
- Responding in role of physiotherapist/exercise physioloist to constructed clinical cases/scenarios including establishing appropriate evaluation criteria (EBL).
05. Apply theories of human behaviour, lifestyle management and communication to enhance interactions with colleagues and clients in the context of clinical scenarios

- Role play; teamwork; peer-review/feedback, online forum (EBL).
