Subject outline

Chronic diseases such as diabetes, asthma, arthritis, osteoporosis, cancer, and chronic cardiovascular conditions are becoming more prevalent in 'aging' societies. In this subject students will develop knowledge and skills in the prevention, treatment and self-management of chronic diseases that reflects the National Chronic Disease Strategy. An interprofessional, person-centred and evidence-based approach will be adopted.

Activities:
In teams, complete critical appraisal of state, federal and international policy and guidelines related to chronic disease management, and use media for dissemination (EBL).

In teams, explore interdisciplinary care as defined by policy documents (see above) (EBL).

03. Critically analyse and reflect on partnerships of care

Activities:
In teams, explore partnerships of care as defined by policy documents (see above) (EBL).

04. Research and critically analyse evidence and theories to develop chronic disease management plans in response to authentic case scenarios

Activities:
Responding in role of physiotherapist/exercise physioloist to constructed clinical cases/scenarios including establishing appropriate evaluation criteria (EBL).

05. Apply theories of human behaviour, lifestyle management and communication to enhance interactions with colleagues and clients in the context of clinical scenarios

Activities:
Role play; teamwork; peer-review/feedback, online forum (EBL).
