REHABILITATION CASE MANAGEMENT

In this subject, students will be introduced to major concepts and theories in rehabilitation and intervention programs. This subject will examine procedures that are useful in rehabilitation when working in health settings. The subject will provide students with an understanding of effective case management skills which leads to proper care coordination, seemless service provider cooperation and the development and delivery of comprehensive rehabilitation services to those living with chronic conditions or permanent impairments.

Graduate capabilities & intended learning outcomes
01. ) Identify and discuss the basis principles of the Case Management process
- Viva voce - Students will be required to complete in class activities and online tasks based on information about case scenarios.
02. ) Apply, demonstrate and report the Case Management principles in relation to practical case studies.
- Viva voce - Students will be required to complete in class activities. Students will apply, demonstrate and explain the principles of rehabilitation Case Management in a practical setting
03. ) Critique and interpret major models relevant to Rehabilitation Case Management and support findings with practical examples in the rehabilitation setting.
- Group presentation: Summative assessment. In groups, students will be required to apply models to a health and human service setting and generate an appropriate rehabilitation plan and resources to support effective Case Management
04. ) Analyse theoretical models and determine the most appropiate assessment tools to create and evaluate a rehabilitation intervention plan.
- Written assessment - Summative assessment. Students will be required to explain and analyse one theoretical model in relation to a case scenario and create and hypothesis the most appropiate assessment tools to generate an appropiate intervention and plan delivery.
