In this subject, you will integrate core strands of theory knowledge and practice with contemporary health needs of individuals, groups, communities and populations. You will develop knowledge and skills in interventions and management plans for people with chronic conditions in both group and individual contexts as well as health leadership and change management skills. You will critique models of health, theories of health behaviour and apply innovative problem-solving skills to health-related scenarios covering a range of chronic conditions. Principles of ethical leadership, change management, social justice, cultural competence, interprofessional, person-centred, evidence-based practice and critical reflection approaches will be adopted during the exploration of topics.
