In this subject, students will be introduced to major concepts and theories in rehabilitation and intervention programs. This subject will examine procedures that are useful in rehabilitation when working in health settings. The subject will provide students with an understanding of effective case management skills which leads to proper care coordination, seamless service provider cooperation and the development and delivery of comprehensive rehabilitation services to those living with chronic conditions or permanent impairments.

01. Identify and discuss the basic principles of the Case Management process, including its application in different cultural groups and in diverse communities
02. Apply, demonstrate and report Case Management principles in relation to local, national and international practical case studies.
03. Critique and interpret major models relevant to Rehabilitation Case Management and apply to practical settings.
04. Analyse theoretical models and determine the most appropriate assessment tools to create and evaluate a rehabilitation intervention plan.
