Subject outline

There is extensive evidence that the successful management of a person's chronic condition is highly dependent on the self management skills of the client themselves. In this web based subject, students will explore case studies and will gain the knowledge needed to support clients to develop the attitudes, behaviours and skills necessary for successful chronic condition self management. Students will cover the underlying principles, barriers and enablers for effective self management. Through guided activities, students will gain skills in collaborative, interprofessional, self management care planning, and interventions and strategies including health coaching.

01. Explain the theory behind and evidence for chronic condition self management

Activities:
Each ILO aligns directly to each of the four sections. In each section the student will be introduced to the content via a case scenario that is supported by audiovisual material, including digital stories. Students are provided with material to stimulate their learning and are encouraged to read widely. The assessment focuses on the student#s ability to demonstrate that they have met each of the ILOs. The case study draws all sections together with students required to develop a self-management plan.

02. Work with health professionals from other disciplines to support chronic condition self management

Activities:
Each ILO aligns directly to each of the four sections.
