As an acknowledgement and thank you for their support, staff who contribute will receive a giving pack that includes a golden lanyard.
By submitting this form I hereby request La Trobe University to commence the workplace giving deduction from my gross salary as a donation to my nominated fund. I acknowledge that La Trobe University will also adjust the amount of tax it withholds from my fortnightly pay to take into account my donation, so I will be paying a reduced amount of tax on my salary. I understand that the deduction will not offset against my gross salary and will appear on my income statement as ‘workplace giving’. I acknowledge that the amount I have indicated above will be deducted regularly each fortnight from the next applicable pay period.
