The Inner East Primary Care Partnership's (IEPCP) Social Inclusion for Older People initiative builds community connections and creates a greater sense of community inclusive of age, ability and cultural diversity. The project has documented: Models of good practice for improving social inclusion for older people from CALD backgrounds; the achievements, strengths, weaknesses and key learnings of the IEPCP Social Inclusion Initiative projects; a âSocial Inclusion Modelâ to work with socially isolated groups; key factors for successfully transferring the Social Inclusion Model more broadly both within the IEPCP and beyond.
