Evaluation of the HARP-CDM interRAI Home Care Pilot

An evaluation to determine the relative efficiency gains and effectiveness of a standardised comprehensive assessment being used (in an electronic environment) in the Hospital Admission Risk Program (HARP) Chronic Disease Management (CDM) service. The outcome will be a report that presents an outline of the background to the evaluation, the evaluation methodology, major findings, recommendations (supported by evidence) to improve models of practice, and advice on system-wide implementation of the interRAI in HARP-CDM services in Victoria
