Here is a list of the various forms for requesting services and resources from La Trobe University (LTU) Library.

Contacting the Library

Issues and general questions

Contact the Library: This is the general contact form that can be used if you have a question or need assistance.
(An online chat is also available during online service hours.)
Report a problem: This form is used to report any issues logging in to a Library system or accessing a resource.
Provide feedback: Use this form if you would like to make a suggestion or provide feedback (positive or negative) about any of the Library's services, facilities or resources.

Requests

Request an alternative format resource: Use this form to request a copy of a resource in an accessible format (e.g. large print).
Submit a Library research request: Use this form to ask a research-related question or request a consultation with a Research Librarian, research training, a Research Attention Value and Engagement (RAVE) report or a customised search.
Submit a teaching support query (LTU staff/academics only): Use this form to ask a teaching-related question or request teaching support from the Library.
Request permission for recording and photography in the Library: Some filming and photography in the Library requires permission. Use this form to request permission.

Refer a student to expert support (LTU staff/academics only): If you are an LTU staff member or academic, you can use this form to refer a student for expert one-to-one advice from an Academic Skills Advisor, English Language Advisor or Librarian.

Reading lists

Submit a reading list query (LTU students): Use this form if you have any questions or experience issues accessing an item on your reading list.
Submit a reading list request/query (LTU staff/academics only): If you are an LTU staff member or academic and require a new reading list, changes to an existing list, have any questions or are experiencing any issues with reading lists, use this form to contact the Library.

Copyright

Submit a copyright query: Use this form if you have any questions or require further information pertaining to copyright.
Make a Copyright Permission Service request: Use this form if you require permission from a copyright owner to use their material in any teaching, learning or research activity in your capacity as a La Trobe University staff member or as a Graduate Research student for use in a thesis, research project or published paper.

Open access publishing

Contact the eBureau team: Use this form if you have any questions about an eBureau publication.
Expression of interest - Publishing with eBureau (academics): Use this form if you are considering publishing with eBureau or have some questions about their publishing process.

Other forms

Register for remote delivery: If you satisfy the eligibility criteria for remote delivery and require physical items from the Library to be posted to you, you can register for our remote delivery service.
Suggest an item for the Library to purchase (LTU staff/students only): If you know of a good resource that isn't available through the Library, you can suggest that we purchase it to add to our collections.

Alumni Library membership form: For LTU Alumni to request to join the Library.

Wodonga TAFE students/staff membership form: Membership form for Wodonga TAFE students and staff to join the David Mann Library to enable borrowing and requesting of physical items. There is also a printable version of this form [PDF 111KB].

CAVAL Library membership form: For students or staff members of an institution listed on CAVAL's list of participating libraries to request to join the LTU Library.

VCE Mildura membership form [PDF 122KB]: VCE/HSC students and staff based in Mildura will need to fill out this form and have it signed and verified to join the LTU Library.
