There is extensive evidence that the successful management of a person's chronic condition is highly dependent on the self management skills of the client themselves. In this web based subject, students will explore case studies and will gain the knowledge needed to support clients to develop the attitudes, behaviours and skills necessary for successful chronic condition self management. Students will cover the underlying principles, barriers and enablers for effective self management. Through guided activities, students will gain skills in collaborative, interprofessional, self management care planning, and interventions and strategies including health coaching.
