Family inclusive approaches in the context of a dual diagnosis: A systematic review

Abstract
Objective: Ill mental health and alcohol or other drug problems go hand in hand for some, resulting in often complex presentations requiring specialised approaches. This review will summarise recent knowledge around the use of family inclusive approaches in the treatment of patients with a dual diagnosis.
Method: A systematic search of five databases from 2000 to 2024 yielded 11,632 records, with 26 meeting inclusion criteria.
Results: Included studies reported on two distinct cohorts: adolescents, presenting with a mental health diagnosis and substance use and adult clients, predominantly in hospital care due to a severe mental health condition paired with serious substance use of often more than one substance. Results for both cohorts revealed that for most studies family inclusive practice performed as good or better than treatment as usual for underlying conditions but often resulted in positive changes in global and family functioning (adolescents, adults), family conflict, parental skills and family communication (adolescents).
Discussion: The intertwining of two health conditions is particularly challenging for practitioners, families and clients alike but family inclusive interventions can not only improve the relational wellbeing of clients and their families but also have a positive impact on both the mental health and substance use related conditions. By identifying effective ways of integrating families in the support and highlighting gaps and further needs this presentation will assist the field to understand the impact family inclusive approaches can make in the life of those facing complex health challenges.

Sandra has 25 years of experience in alcohol research and has repeatedly developed and evaluated health promotion and prevention programs. A central topic of her recent research is the impact parents have on their children’s alcohol-related knowledge as well as the role alcohol plays in the work-life balance of parents.
