Report all losses to authorities (police, hotel, airline, hospital, Supervisor, Coordinator) and the Insurance Office as soon as possible.
In order to submit a claim to the Insurance Office at La Trobe University, please complete the relevant Travel Claim form:
Email your completed claim form and supporting invoices/receipts to email@example.com within 30 days of the incident date.
You will be required to submit documentation to support your claim. This may include:
- police report
- medical report
- written confirmation of loss
- statutory declaration.
