Cross-overs, stepped wedges and staircases: some recent work in longitudinal cluster randomised trials

Abstract:

Although individually randomised trials are the gold standard for assessing the  impact of new treatments on patient outcomes, cluster randomised trials  are necessary when testing the effect of healthcare provider-level changes on patient  outcomes, e.g. the effect of a hospital-wide handwashing program on the  number of patients who acquire infections in hospital. Cluster  randomised trials often require large numbers of clusters and thus can be infeasible, but longitudinal cluster randomised trials, where clusters may switch between intervention and control, require  smaller sample sizes. Cross-overs, stepped wedges and staircases are all  particular variants of longitudinal cluster randomised trials that are being conducted with increasing frequency.  However, many of the underlying statistical aspects of these designs  remain under-explored.
