This form is required for CANCELLATION TO CLASSES ALREADY BOOKED, so that they can be modified on the University Timetable. This is not to be used for adding new or additional classes.

Please complete the details below and submit the form. It will automatically be posted to the Timetable Co-ordinator in the Timetable Office and a confirmation will be sent to you.
