Adverse Incidents : reporting responsibilities

Investigators are required by law to "promptly notify the Animal Ethics Committee of any unexpected or adverse effects which occur during the period of the approved project and which impact on the welfare of the animals", Clause 3.1.12 of the 7th Edition of the Code. An incident report form is available.
