Before completing an application form, investigators are asked to read the guidelines and instructions outlined below.

Applicants are asked to use the most recent version of the application form where possible. The most recent versions can be found below.

Please save a copy of the relevant form to your computer and use this copy to complete your application. If you have any difficulties please contact our office.

Applications can be submitted at any time. However, applications should be submitted no later than at the designated submission date to be considered at a particular meeting.

- SOP Proforma if you want to create standard operating procedures
- Fieldwork Notification Form if you want to conduct fieldwork in Victoria and outside La Trobe University licensed premises
