Application Procedures, Forms and Reporting

 

To assess the level of risk within your proposal, please check the Human Ethics Risk Assessment Checklist. Warning to Mac users - application may unexpectedly close and not save content.

Before completing an application form, you are advised to review the four principal values of the National Statement (1.1 - 1.13) and their relevance to your proposal.

Please ensure that a current application form is used. Committees may return applications prepared on outdated forms.

When planning project schedules, please allow a minimum of 30 working days for above-low risk proposals to receive a response letter from the UHEC.

Special Notice - All approved studies are now required to state their La Trobe ethics application reference number on the Participant Information Statement and on all recruitment documents.  Section 8(e) of the Guidelines for Completion of the Application Form and the Guidelines for the Participant Information Statement have been updated to reflect this new requirement.

Reminder - Progress Reports for all UHEC/FHEC approved studies are due by 12 February

  • Ethics - Application for Modification to Project: submit to either UHEC or relevant FHEC depending on where the project has been approved.
  • Ethics - Progress/Final Report Form
  • Adverse Event Form. Note that researchers are required to report adverse events or outcomes promptly to the institution (Section 5.5.3 of the National Statement), in this case the La Trobe University Human Ethics Committee or the appropriate Faculty Human Ethics Committee.
  • Human Ethics Complaints Form (doc 35 kB). Note that it is a requirement of the La Trobe University Human Ethics Committee to handle complaints as outlined in Chapter 5.6 of the National Statement (2007).
