Application Forms and Reporting

 

Before completing an application form, please review the La Trobe Human Research Ethics Guidelines and undertake a risk assessment of your research project using the Human Ethics Risk Assessment Checklist. The Guidelines offer information about ethical requirements and the classifications of negligible risk, low risk and above-low risk research. The Checklist helps you identify which application form to use. It is also recommended that you review the four principal values of the National Statement (1.1 - 1.13) and their relevance to your research proposal.

Always use a current La Trobe application form - download from menu below - or you are welcome to submit using the National Ethics Application Form (NEAF).

Above-low risk proposals need to allow a minimum of 30 working days before receiving a first response from the UHEC.

Special Notices -

All continuing research studies must submit a 2012 Progress Report which were due by 12 February 2013. If you have not yet submitted a report, it may result in suspension of ethics approval.
