Policy and Procedures relating to Statement of Authorship

Each assessment item is to have attached to it a completed School of Public Health "Assignment Submission Form". This form contains a "Statement of Authorship" which must be completed and signed in respect of each item of work submitted.

Statement of Authorship for Theses

The Research and Graduate Studies Committee and Academic Board expect that all theses and research reports submitted as part of the assessment for Masters by Coursework, Honours Degrees, Postgraduate Diplomas and Graduate Diplomas include a signed and dated declaration that either none of the research undertaken required approval by a University Ethics Committee or that the research undertaken was approved by a University Ethics Committee. In the latter case, the declaration should state the ethics approval number for the research project.

Statement of Authorship forms are available from the above website for both individual authorship and group authorship.
