VERRUCAE
 

-A benign viral tumour (wart)

 


• Human papilloma virus (HPV)

 


o common warts (verruca vulgaris)

 


o plane warts (flat warts)

 


o plantar warts

 

 




 

VERRUCA PEDIS (Papilloma)

 


o viral inoculation

 

o peak incidence = young adolescence

 

o Commonly spread via communal activity

 

o Auto-inoculation

 

 

 

 

 


Histopathology

 

o Increased mitosis

 

o Exaggerated dermo-epidermal junction

 

o Capillaries drawn toward epidermis

 

o Black specks which bleed on debridement

 

 

 

 

 

 


Clinical presentation

 

o Size & shape can vary

 

o Single, multiple or mosaic

 

o Any surface

 

o Overlying HK

 

o Disruption of skin striations

 

o Pain on medial & lateral compression

 

o Pin point bleeding on debridement

 

 

 

 


Immune response and regression

o Estimated 20% resolve in 6 months with majority resolving within 2 years

 

 

 

Treat or not to treat

o Site, size, chronicity, patient lifestyle, patient age.

 

Success depends on:

o Patient age, type, number, duration, host factor

 

 

 

 


 

Verruca Management



Chemical destruction:

 

 

• Salicylic acid

 

 

• Lactic acid

 

 

• Trichloracetic acid

 

 

• Monochloracetic acid

 

 

• Potassium hydroxide

 

 


• Silver nitrate

 

 

• DNCB

 

 

 

 


Freezing agents (cryogens)

 

• Liquid nitrogen

 

 

• Carbon dioxide snow

 

 

 

 

 


Use of heat

• Electrodessication

 

     o fulguration

 

     o desiccation

 

     o coagulation

 

     o electrosection

 

 

 

 


Physical removal
