Psoriasis

 

 
 

A chronic, non-infectious, inflammatory dermatosis


• Females>males

 

• Genetic predisposition

 

• Environmental factors

 

• ‘Koebner’s phenomenon’

 

 

 

 


 

Clinical appearance

 

• Well-demarcated plaques

 

• Topped by silvery scales

 

 

 


Clinical varieties

 

• Psoriasis vulgaris

 

o Lesions sharply defined, raised, erythematous with silvery white scales

 

o Common on knees and elbow

 

o Auspitz’s sign

 

 

 

 


• Psoriatic keratoderma

 

o Severe plaques

 

o HK, erythema and deep fissures

 

 

 

 


• Pustular psoriasis

o Generalised

 

- Small, sterile, yellowish pustules develop on erythematous background

 

- Rare, can be fatal

 

 

 

o Palmoplantar

 

- Discoid, red and scaly lesions with small pustules

 


 

 

 

 

 

 

 

• Psoriatic nails

 

o Pitting or stippling

 

o Hypertrophy or discolouration

 

o Onycholysis and shedding

 

o Parakeratosis

 

o Subungual HK

 

o Erythema, swelling and desquamation of periungual tissue

 

 

 

 


• Psoriatic arthropathy

 


o Sero-negative (RA factor), inflammatory arthropathy

 

o DIPJ involvement

 

 

 

 



 

Treatment

 

• Topical corticosteroids

 


• Coal tar

 

• Dithranol

 

• Cytotoxic drugs

 

• PUVA

 

• Emollients

 

• Retinoids

 

• UVB light therapy
