HAV - Surgical Correction

Surgical correction of HAV is quite common and there are numerous methods used. Hallux abducto valgus surgery can be broken down into two types of surgical procedures, those that preserve or reconstruct the joint and those that remove or remodel the joint.

 

Case 1: Austin (Chevron) "V" osteotomy which realigns the joint with a lateral shift of the metatarsal head via a distal osteotomy. K-wire fixation is shown. Note the position of the sesamoids.

Case 2: Keller arthroplasty, with resection of the base of the proximal phalanx. This can lead to ongoing instability even with soft tissue reconstruction of the joint (as this case shows).  

Post HAV surgery (psuedo joint)

Lateral Oblique View- Distal metatarsal osteotomy with proximal hemiphalagectomy. The joint space is maintained with two Oblique K-wires allowing the formation of a psuedo joint.
