Enthesopathy

Enthesopathy is mechanically induced heel pain which is localised to the calcaneus. Constant stress from the plantar fascia at the site of its insertion results in enthesitis. Progresion results in new bone formation resulting in a 'spur'. They are quite commonly asymptomatic.

 

Small calcaneal spur on its plantar aspect at the insertion of the Plantar Aponeurosis; best viewed on lateral radiograph.

 

Large calcaneal spur; there is also calcification at the insertion of the Achilles tendon.
