On-Line Incident and Hazard Reporting

La Trobe University has developed an On-line Incident and Hazard Reporting System which enables tracking of reported incident and hazards as well as the actions to control the risks. The System also enables supervisors to enter and track corrective actions following a report.

Incidents of a confidential nature (for example, psychological injury) may be reported directly to the Workplace Environment and Wellbeing Unit by contacting the Unit on 9479 1186 (all campuses except Bendigo ) or 5444 7901 ( Bendigo ).

The following information is to assist people in managing an incident or injury in the workplace.

If there is an immediate risk to the health and safety of persons, the University emergency response procedures must be followed. Report the emergency via the campus emergency number and provide details about the location and nature of the emergency. Security will arrange for the appropriate emergency services to attend and will take charge until the emergency services arrive.

If any persons have been injured, arrange for a first aider to administer first aid if appropriate. The contact details of designated first aiders are normally displayed at school and department reception areas.

If the incident is Notifiable under the Occupational Health and Safety Act 2004, the Occupational Health and Safety Section must be informed immediately on 9479 1186 (all campuses except Bendigo) or 5444 7901 or 7472 (Bendigo). Incidents occurring after normal business hours must be reported immediately to Bundoora Security on 9479 2012.

The workplace where the incident occurred must not be disturbed until WorkSafe attend or advise otherwise, unless disturbance of the workplace is necessary to protect the health and safety of persons, aiding injured persons involved in the incident, or taking essential action to make the site safe or to prevent a further occurrence of an incident.

Report all incidents and injuries, via the Online Incident Report to the Workplace Environment and Wellbeing Unit as soon as possible. All employees, or someone on their behalf, must report in writing any work related injury or condition within 30 days of becoming aware of it.
