Services and Eligibility

Clients will be admitted to the clinic if they meet the eligibility guidelines outlined in the above documents and agree to the fee schedule and attendance requirements.

Fees vary according to the service offered and funding source.


Making an Appointment

For enquiries or to register for services please contact the Clinic reception on (03) 9479 1921 between 9:00 - 5:00 pm Monday to Friday. Prospective clients will be sent a Consumer Information Form and LCC Consent Form within two working days of initial enquiry. These forms must be completed, signed and returned to the clinic before an appointment can be made.

Attendance

Clients will be discharged from the clinic if they are unable to meet the attendance requirements. Clients will be discharged if they:

  • Fail to attend two appointments without prior cancellation
  • Cancel three appointments
How are services arranged?

Once the Consumer Information Form is received, clients are allocated to the relevant service. If additional information is required staff in the clinic will contact the prospective client. The intake worker will review all Consumer Information Forms weekly.
