Clients will be admitted to the clinic if they meet the eligibility guidelines outlined in the above documents and agree to the fee schedule and attendance requirements. FeesFees vary according to the service offered and funding source.
Making an AppointmentFor enquiries or to register for services please contact the Clinic reception on (03) 9479 1921 between 9:00 - 5:00 pm Monday to Friday. Prospective clients will be sent a Consumer Information Form and LCC Consent Form within two working days of initial enquiry. These forms must be completed, signed and returned to the clinic before an appointment can be made. How are services arranged?Once the Consumer Information Form is received, clients are allocated to the relevant service. If additional information is required staff in the clinic will contact the prospective client. The intake worker will review all Consumer Information Forms weekly. Other Important Information
