Registering for disability services
To receive support you need to register in the Equality and Diversity Centre. You will need to do the following:
- Complete the following forms:
- Personal details form (PDF 38KB) (Word 216KB)
- Confidentiality agreement (PDF 46KB) (Word 217KB)
- Obtain current supporting documentation from a relevant Health Practitioner that complies with the Documentary Evidence Guidelines (PDF 50KB) (Word 216KB) and provide either:
- a comprehensive letter (on official letterhead) explaning the disability or medical condition, the impact on studies and recommendations on any services you require
OR - Complete the Health practitioner's statement (PDF 65KB) (Word 239KB)
- a comprehensive letter (on official letterhead) explaning the disability or medical condition, the impact on studies and recommendations on any services you require
- Make an appointment to meet with a Disability Liaison Officer to discuss your requirements.
