Request for Student Feedback on Subject
Note: Please submit the request for a Student Feedback on Subject (SFSU) form at least 10 working days prior to your planned administration date. If the Feedback Team receive requests with less advance notice, the CTLC cannot guarantee provision of forms by your requested date.
Who can request a SFSU form?
- A Faculty, school or department nominated representative can initiate a request for a SFSU form on behalf of a permanent, fixed term or casual lecturer.
How do I request a SFSU form?
- The Faculty, school or department nominated representative should email student.feedback@latrobe.edu.au to action the request.
- The Feedback Team will acknowledge receipt by return email and make contact to complete the request.
What happens next?
- Most elements of the SFSU form are set. On the front page there are two set open-ended questions; one additional open-ended question can be specified. On the back page there are 15 set fixed response statements; up to five additional fixed response statements can be specified (view sample SFSU form). A member of the Feedback Team will consult with the Faculty, school or department representative to specify the additional questions and statements and create the SFSU form.
Note: All subject feedback forms requested for subjects taught using online/distance technologies must include one additional question on the effectiveness of that technology. Consequently, those subjects will have a total of four optional questions to choose from, not five.
- The SFSU master form and instructions for administration will be sent via LTU internal mail. Please follow these instructions carefully.
