  A Way Forward for Consumer, Carer and Community Engagement in Primary Health
 
A project to undertake a review of present practices and to develop recommendations about how the Primary Health Branch can continue to support effective consumer, carer and community participation in the primary care sector until 2010.

  Aged Care and Community Care Services Review Project
 
A project to develop a preferred information model for nominated community and aged care services including integration with noncommunity and non-aged care information management systems; and provide detailed specifi cations for the implementation of the community and aged care services information management system.

  Assessment to Service Outcomes – Care Pathways for Older Australians with Dementia, Cardiovascular Disease and Arthritis
 
The project is using existing administrative by-product data and apply established data linkage expertise (at the Australian Institute of Health and Welfare) to explore the care transitions and pathways for older Australians diagnosed with one of three major chronic diseases that contribute substantially to the burden of disease in Australia, namely: Dementia, Cardiovascular Disease and Arthritis and Musculoskeletal conditions.

  Australian Government Dementia Health Priority Initiative – National Evaluation Project
 
The national evaluation is a three- year project to evaluate the Dementia Health Priority Initiative. The national evaluation is being conducted by AIPC in collaboration with The University of Melbourne, Applied Aged Care Solutions and Access Economics, under the direction of Dr Colleen Doyle from AIPC.

  Child Youth and Family Audit Tool
 
This short-term project developed an audit tool for use as both an internal and external assessment document for community service organisations that are to be registered under the Children Youth and Family Act 2005. The project had three key deliverables: develop an audit tool to support the internal quality improvement reviews and external quality assurance audits of 105 community service organisations providing family and out-of-home care services; provide advice on sampling to the Department of Human Services regarding the registration process; conduct training in basic CQI practice and in the use of the audit tool.

  Design and Monitoring an Evaluation for the Australian Better Health Initiative
 
In collaboration with the School of Public Health, a project to provide a monitoring and evaluation framework for the Australian Better Health Initiative that will focus on key strategy features of the program to ascertain its effi ciency, appropriateness and effectiveness across Australia, while allowing for coordinated evaluation efforts on particular program activities at the state and local levels.

A project that developed an evaluation framework for implementation, establishment and review of Child and Maternal Health pilot projects. The aim of the evaluation framework was to determine whether the aims and objectives of the pilot projects have been met.

  Evaluation of National Drug Centres of Excellence
 
This project focused on Australia’s national centres of excellence on drug use: the National Drug and Alcohol Research Centre; the National Drug Research Institute, and the National Centre for Education and Training on Addiction. The main purpose of the project was to ascertain the effectiveness of the research centres in the provision of quality research and products that contribute to evidence- informed practice by health, law enforcement and education services. Mechanisms to enhance the provision of this research were identifi ed and progress against findings of the 2003 evaluation was ascertained. The appropriateness of infrastructure funding arrangements for each research centre was examined.

  Evaluation of the Arthritis and Musculoskeletal Conditions Quality Improvement Program
 
A project to provide the Department of Health and Ageing with a comprehensive evaluative report on the activities and outcomes of each project funded under the Arthritis and Musculoskeletal Conditions Quality Improvement Program (AMQuIP) against the projects’ and programs’ aims and objectives; and a comprehensive evaluative report on the overall performance and outcome of the Arthritis and Musculoskeletal Conditions Quality Improvement Program, against the program’s aims and objectives.   Evaluation of the Aged Care Assessment Program
 
As the Victorian state Evaluation Unit for the Aged Care Assessment Program (ACAP) since 1983, the Lincoln Centre oversees the collection of the Minimum Data Set (MDS) on the approximately 50,000 assessments carried out each year by Victoria’s 18 Aged Care Assessment Services (ACAS).

  Evaluation of the Eastern Health HARP and HDM Projects
 
A project to ensure routine and systematic application of standardised evaluation principles and practices to projects, support Eastern Health HDM projects in rigorous and effective evaluation, assist Eastern Health to establish a culture, systems, and processes that value, support and facilitate evaluation practice, and enable clear conclusions to be drawn regarding outcomes of project implementation.

  Evaluation of the School Focused Youth Service
 
An evaluation of the SFYS, a jointly funded initiative of the Department of Human Services and the Department of Education and Training, that assessed the impact of the SFYS program on schools and young people in the program’s target group, assess the effectiveness of the SFYS in facilitating and strengthening partnerships between schools and community sector agencies with a view to supporting young people; receive advice and provide recommendations in regard to enhancement of the current service delivery model; assess the value-add of the SFYS program to other current DHS and DE&T initiatives.

  Home and Community Care Active Service Model Project: Literature Review and Identification of Service Models
 
The outcomes of the project will be: A comprehensive evidence base to support the case for change from a dependency model of service delivery to an approach that includes person-centred capacity building or restorative care to maintain or restore client capacity to live as independently as possible in the community; Engagement of the HACC sector in Victoria in significant changes to policy and practice towards an Active Service Model; Identification of models of service delivery that are applicable in Victoria, including identifi cation of suitable target groups within the HACC target population, having regard to how HACC operates in Victoria and the aims of the Active Service Model Project; and, recommendations for implementing person-centred capacity-building and restorative care approaches across the HACC service system in Victoria, with particular attention to funding policies and the readiness of the service sector.

  Home and Community Care National Standards Assessment Program for HACC-funded Agencies Accredited with Quality Improvement and Community Services Accreditation (QICSA)
 
This project aims to develop and implement an integrated assessment process for assessment against both Quality Improvement Council (QIC) Standards and Accreditation program and the Home and Community Care (HACC) National Standards Assessment program for HACC-funded organisations that are QIC-accredited through Quality Improvement and Community Services Accreditation (QICSA).

A project to improve the safety and quality of PDRSS through the provision of training and support for effective governance and continuous quality improvement. The project also aimed to contribute to strategic quality planning for the sector.

  Improving the Culture of Hospitals
 
Tools and processes will be developed that will enable hospitals to engage in a process of cultural reform to make the acute care experience more culturally acceptable for Aboriginal patients and communities. The tools and processes will reflect the Plan Do Study Act method commonly used in acute care facilities in this country. The primary research method will be case studies with interventions in 16 hospitals nationally. The project will also conduct a concurrent training program for Aboriginal people who will participate in the cultural improvement processes.

An examination of ways that funding of chronic disease care and individual prevention activity in primary medical settings could improve the overall efficacy and appropriateness of chronic disease management. Queensland Health
  National Data Repository for the Aged Care Assessment Program
 
A national data repository, established in 2003, responsible for the collation and validation of the ACAP MDS from Evaluation Units in all Australian states and territories. The data are used to monitor trends in numbers and characteristics of ACAS clients and to assist in program evaluation and planning of services for older people.

  Pilot of the Victorian Quality Council’s Safety and Quality Education Program
 
A project to pilot the provision of the VQC’s Safety and Education Program to four teams of 6-8 participants, at three Victorian sites selected from metropolitan, regional and rural Victorian health services. The key service areas included: central coordination of the program; maintenance of the online component of the course; and provision of a number of expert facilitators to undertake the workshop and provide feedback to participants. Changes required to the program to assist with accreditation at university level were identified.

  Primary Health Care Consumer Opinion Survey
 
The surveying of consumers at Victorian community health centres project was a two-year state-wide quality improvement initiative funded by the Primary and Community Health Branch of the Department of Human Services. It aims to assist in evaluating and improving services to consumers at CHCs by asking them about their level of satisfaction on three main domains: centre environment, service provision, and degree to which special needs are met by the CHC.

  Provision of Audit and Audit Support Services
 
A project to establish an effective accreditation program for children’s and youth services. Australian Council for Children & Youth Organisations Review of Radiation Oncology Standards A review of the newly developed package of standards. The review included: application of consistent terminology; development of framework and organisation of material; appropriate improvement in readability; inclusion of relevant cross-referencing; and development of presentation format.

  Review Pilots of the Statewide Homelessness Assessment and Referral Framework (SHARF)
 
A review of the pilots of the Statewide Homelessness Assessment and Referral Framework. The Framework aimed to achieve a better response to people seeking assistance from homelessnessfunded services by increasing the efficiency of the existing homelessness service system and by supporting initial assessment and referral practice as a distinct skill.

  Review to Identify Opportunities to Improve Outcomes for Aboriginal People
 
A review to assess the Department of Human Services’ current portfolio of programs, in relation to their ability to contribute to the performance framework of the Victorian Indigenous Affairs Framework; to investigate practices and policies in other jurisdictions; to identify opportunities for DHS to create a more effective match between funding and improving outcomes for Aboriginal people in Victoria.

  Service Model Evaluation of Drugs and Alcohol and Mental Health Treatment
 
Research to determine good practice and facilitate improved treatment outcomes for people with co-existing mental health and substance abuse problems; and increase understanding of the impact of service structure on co-morbidity treatment outcomes, as well as develop good practice models to ensure improvement in co-morbidity treatment service delivery.

  Societal and Economic Burden of Chronic Non-communicable and Communicable Disease in Victoria
 
A review of Australian and international literature on the societal burden of chronic disease with reference to its relevance to public health policy in Victoria; a review of the economic impact of health inequalities as relevant to socioeconomics of the developed world. A model/methodology for the economic impact of health inequalities due to socioeconomics (in collaboration with DHS); and, estimates of the costs (health, social services and productivity) of health inequalities due to socioeconomics in Victoria.

  A project to conduct a stock-take and review of drug prevention research relevant to the Indigenous community and published over the past five years in Australia, with particular attention to the Victorian context.

  Systematic Review of the Delivery of Services to Victorians with a Spinal Cord Injury and Development of Strategic Framework
 
A strategic framework that addresses projections for future service demands and achievement outcomes; recommendations to enhance the relevance, quality and accessibility of rehabilitation and community integration services available to people with spinal cord injury; ways in which existing acute, sub-acute and community services could be re-figured to meet needs more effectively and reduce duplication; specifi c opportunities arising from improved coordination of existing services across the sector; and additional services required to fi ll the gaps.
